The form outlines in an easy, matter-of-fact, common-sense way all the decisions you might want to make now - about the type of Service and where you'd like it held, about choices such as burial or cremation, necessary vital statistic information, and any particular concerns or considerations you may have.

The original is returned to us and you keep a copy for your family or executor. You'll also receive a laminated wallet-size card giving our 24-hour phone number, and indicating your final wishes are on file.

You can fill the forms out online or download them for fax or mail-in. Just select the location nearest to you.
